During this initial, electronic consultation, you will be pre-screened for our weightloss program and or aesthetic procedures. A determination will be made on eligibility within 48 hours of submitting the documentation. 

This does not guarantee eligibility to participate in weightloss or treatments with Lush and Luxe. *A variety of medical conditions could exclude you from being eligible for the therapies offered with Lush and Luxe. This will be determined on an individual, case by case basis. The determination is made by the medical provider and is final. Consultation fees are non-refundable. 

This consultation time can be utilized for any service at Lush and Luxe.
