Price is a 3 month commitment package $1200.00 and $50.00 for GFE (good faith exam).  $50.00 for GFE (good faith exam) is non-refundable and is not applied to package.  Injections are weekly. What to expect: Your weight, measurements and blood pressure will be taken at your initial appointment in addition to your first injection of Semaglutide and B12. Initial dose and subsequent doses are determined by Dr Chertoff following his treatment plan. There are no additional fees for Semaglutide & B12 weekly injection appointments these are part of your Semaglutide Initial Appointment Package.

Your consultation will allow us to see and hear your personal concerns to tailor a treatment plan with your goals in mind.  The consultation includes discussion of your treatment plan, which includes the cost and any financing options you might be interested in. The $50.00 consultation fee will be applied to your chosen service. If you are unable to keep your scheduled appointment or if our office is notified less than 24 hours of your cancellation or reschedule, you will be charged the consultation fee and it is NONREFUNDABLE. NO REFUND of consultation fee will be issued for no-shows.
